It is important to review your personal medications with your pharmacist to ensure you are receiving full benefit of your medication to maintain and improve your health.  It is a good idea to create and update annually a best possible medication record during a medication review especially if you take several medications each day. It is particularly important to arrange a medication review/update with your pharmacist if:

  • You are planning to be admitted to hospital (in order to have an up-to-date medication record to share with hospital staff)

  • You have been discharged from a hospital (in order to update your medication record to reflect any changes made in hospital)

  • A review has been recommended by your doctor or other healthcare providers. A current list of your prescription and non-prescription medications (including vitamins and herbals) may assist your family doctor, specialist(s) or registered clinical nurse to provide you with the best possible care. 

  • You are having difficulty taking your medications regularly. Your pharmacist can suggest strategies to help you. 

  • You have questions or concerns about your medications. Your pharmacist can work together with your doctor to discuss any issues you may have.
